The Referral and Linkages coordinator will be based in Lwengo/ Lyantonde districts and will be responsible for referral and linkages component support of KCHS activities in two districts (Lwengo and Lyantonde), S/He will be responsible for working closely with Health units, supervising referral and linkages facilitators and supporting community-health facility linkages for children, adolescents living with HIV/AIDS, children affected by violence and their families. He/she will be responsible for supporting facility-community referral linkages for comprehensive services package to children and adolescent living with HIV/AIDS and their families, compiling and submitting work plans and reports among others.
Person Specifications
The holder should possess a bachelor’s degree in Social work, Social sciences, Community development, Community health with a postgraduate relevant qualification from a recognized institution and relevant work experience of managing NGOs community-based children and adolescent health projects specifically HIV/AIDS,
